What Is Wisdom Teeth Removal?

Wisdom teeth removal (visdomstannekstraksjon) is the surgical or non-surgical extraction of one or more third molars. These teeth sit at the very back of the mouth and often become problematic because modern jaws frequently lack the space to accommodate them.

There are three common scenarios that lead to removal:

  • Impacted wisdom teeth — the tooth is trapped beneath the gum or bone and cannot fully erupt
  • Partially erupted teeth — the tooth has broken through the gum only partially, creating a flap of tissue that traps food and bacteria
  • Crowding or pressure — the emerging tooth pushes against the second molar, causing pain or shifting

Not all wisdom teeth need to be removed. If they are fully erupted, properly aligned and easy to clean, your dentist may recommend monitoring rather than extraction.

What to Expect

Before the procedure

Your dentist or oral surgeon takes an X-ray (often a panoramic OPG image) to assess the position, root shape and proximity of the wisdom teeth to nerves and sinuses. Based on this assessment, they determine whether the extraction is straightforward or requires a surgical approach.

Simple extraction

If the wisdom tooth has fully erupted and is accessible, the dentist can remove it under local anaesthesia using forceps. The tooth is loosened with an elevator instrument and then extracted. This procedure typically takes 15-30 minutes.

Surgical extraction

For impacted or partially erupted teeth, a surgical approach is needed:

  1. Local anaesthesia is administered to numb the area completely
  2. An incision is made in the gum tissue to expose the tooth and bone
  3. Bone removal — if bone is covering the tooth, a small amount is carefully removed
  4. Tooth sectioning — the tooth may be divided into smaller pieces for easier removal
  5. Extraction — the tooth (or pieces) is removed
  6. Stitching — the gum is closed with dissolvable sutures

The entire surgical procedure typically takes 30-60 minutes per tooth.

After the procedure

You will receive specific aftercare instructions. General guidelines include:

  • Bite on gauze for 30-45 minutes to control bleeding
  • Apply ice packs to the outside of the cheek (20 minutes on, 20 minutes off) for the first 24 hours
  • Take prescribed painkillers — ibuprofen and paracetamol are usually sufficient
  • Eat soft, cool foods for the first 2-3 days
  • Do not smoke, use straws or rinse vigorously for at least 48 hours (to prevent dry socket)
  • Gentle salt water rinses can begin after 24 hours
